High-grade serous ovarian cancer (HGSC) Overview
High-grade serous carcinoma (HGSC) is a type of ovarian cancer where the tumor arises from the serous epithelial layer in the abdominopelvic cavity and is mainly found in the ovary. HGSCs make up the majority of ovarian cancer cases and have the lowest survival rates. High grade serous cancers are the most deadly form of ovarian cancer and account for up to 70 percent of ovarian cancer cases.
In most patients, ovarian cancer is diagnosed when the disease has progressed to an advanced stage, corresponding to stages IIb to IV with the involvement of the peritoneal cavity and other organs. HGSOC is diagnosed at advanced stages in most of cases, and these patients have a significantly worse outcome than those with early stage disease. The majority of high grade serous ovarian cancers have been found to originate in the fallopian tube, not the ovary.
BRCA Mutation in High-grade serous carcinoma (HGSC)
Breast cancer gene 1 (BRCA1) and breast cancer gene 2 (BRCA2) normally help control the growth of cancer cells. Mutations in these genes (which can be inherited, or passed, from either parent to the child) increase the risk for breast cancer. These mutations also increase the risk for ovarian cancer. But not all women with mutations in the BRCA1 or BRCA2 gene will develop ovarian cancer.
Overall, the BRCA1 mutation increases the risk for ovarian cancer more than the BRCA2 mutation. Women with the BRCA1 mutation are more likely to develop ovarian cancer before they are 50 years of age. Women with the BRCA2 mutation are more likely to develop the disease after they are 60 years of age. Ovarian cancer is usually diagnosed at a younger age in women who do not have a BRCA gene mutation.
Ovarian cancer is likely linked to an inherited BRCA1 or BRCA2 gene mutation if the woman has:
- Several family members with breast or ovarian cancer
- One or more female relatives who developed breast cancer before they were 50 years of age
- A relative with both breast and ovarian cancer
- Family members who develop cancer in both breasts (called bilateral breast cancer)
- A male relative with breast cancer
- Ashkenazi (Eastern European) Jewish ancestry
Serous carcinoma (which is a type of epithelial ovarian carcinoma) is more commonly linked to BRCA gene mutations than other types of ovarian cancer. Having ovarian cancer linked to a BRCA gene mutation also increases the risk of developing papillary serous carcinoma of the peritoneum, which is a cancer in the lining of the abdominal cavity.
Women patients with ovarian cancer related to a BRCA gene mutation also have a higher than average risk of developing breast and other cancers. Women patients diagnosed with high grade serous ovarian cancer usually undergo genetic counseling and testing for a specific genetic mutation (BRCA 1 & 2). One in five women with a high grade serous pelvic carcinoma are found to have a genetic mutation in BRCA. The BRCA gene mutation is linked to high grade serous pelvic cancers and breast cancer.
Signs and Symptoms of High-grade serous ovarian cancer (HGSC)
Following are the signs or symptoms of ovarian cancer include:
- Bleeding from the vagina that isn’t normal (such as heavy or irregular bleeding, bleeding between periods), especially after menopause
- Frequent discharge from the vagina that is clear, white or colored with blood
- A lump that can be felt in the pelvis or abdomen
- Bladder problems such as the need to urinate often and the urgent need to urinate
- Constipation
- Changes to digestion such as feeling full after a small meal, loss of appetite, heartburn, gas, indigestion or nausea
- Diarrhea
- Frequent feeling of pressure in the pelvis or abdomen
- Fatigue
- Pain in the legs, lower back, pelvis or abdomen
- Pain when having sex
- Swelling of the abdomen
- Unexplained weight loss
- Buildup of fluid in the abdomen (ascites), around the lungs (pleural effusion) or in the legs (lymphedema)
- Difficulty breathing
- Headaches
- Fever
