Goodpasture’s Syndrome (Anti-GBM disease) Overview

Anti-GBM is a rare disease also known as Goodpasture’s syndrome is an autoimmune disorder in which the body’s immune system creates antibodies that attack the kidneys and the lungs. Anti-GBM disease (Goodpasture’s syndrome) characterized by rapidly progressive glomerulonephritis accompanied by pulmonary hemorrhage.

As a result, a patient may develop glomerulonephritis inflammation of the glomeruli (which are tiny units in the kidney’s that filter wastes and extra fluid from the blood) and bleeding in the lungs, either one or both of these conditions.

Anti-GBM disease most often affects men in their 20s and women in their 60s, but it can occur at any age. The condition can occur in children, but this is extremely rare.

Complications of Goodpasture’s Disease (Anti-GBM disease)

Complications can occur quickly due to Goodpasture’s syndrome without treatment. As the disease progresses, about 80–90% of people with the condition develop kidney glomerulonephritis. This involves inflammation in the part of the kidney responsible for filtering waste.

Kidney inflammation can lead to complications, including kidney failure, which may require short- or long-term dialysis. Typically, under 30% of survivors with Goodpasture’s syndrome require long-term dialysis.

About 50% of people with the condition also experience bleeding in the lungs. This may progress to respiratory failure, which can lead to requiring a breathing tube and mechanical ventilator to assist breathing.

Diagnosis of Goodpasture’s Disease (Anti-GBM disease)

Doctors diagnose Goodpasture’s syndrome according to a person’s symptoms, medical history, physical exam, and diagnostic tests.

A kidney biopsy, when doctors remove a tissue sample, is the gold standard for diagnosing the condition. The presence of anti-GBM antibodies on a biopsy specimen confirms the diagnosis.

Additional diagnostic tests also help rule out other conditions and indicate a need for a kidney biopsy. Tests may include:

  • First, a urinalysis to check for blood in the urine
  • Next, kidney function tests to determine damage

The tests above are less invasive than a kidney biopsy and typically help provide clues to the diagnosis.
