Polymyalgia Rheumatica Overview

In fact, polymyalgia rheumatica is an inflammatory disease most commonly seen in older adults, usually causes pain and stiffness in the shoulders and upper back and the hip area. Polymyalgia rheumatica (PMR) is an inflammatory disorder that causes widespread aching, stiffness and flu-like symptoms. 

Moreover, polymyalgia literally means “many muscle pains.” Rheumatica means “changing” or “in flux.” It is more common in women than men and is seen more often in Caucasians than other races.

Signs and Symptoms of Polymyalgia Rheumatica

  • Depression

Causes of Polymyalgia Rheumatica

Exact cause of polymyalgia rheumatica is unknown. Two main factors appear to be involved in the development of this condition:

  • Genetics: Certain genes and gene variations might increase your susceptibility.
  • Environmental exposure: New cases of polymyalgia rheumatica tend to come in cycles, possibly developing seasonally. This suggests that an environmental trigger, such as a virus, might play a role. But no specific virus has been shown to cause polymyalgia rheumatica.

Giant Cell Arteritis

Polymyalgia rheumatica and an another disease known as giant cell arteritis have many similarities. Many patients who have one of these diseases also have symptoms of the other disease.

Giant cell arteritis results in inflammation in the lining of the arteries, most often the arteries are in the temples. Signs and symptoms include headaches, jaw pain, vision problems and scalp tenderness. If left untreated, this condition can lead to stroke or blindness.

Diagnosis of Polymyalgia Rheumatica

Under the new criteria developed by the American College of Rheumatology and The European League Against Rheumatism, patients of age 50 years and older can be classified as having PMR if they meet the following conditions:

  • Shoulder pain on both sides
  • Morning stiffness that lasts at least 45 minutes
  • High levels of inflammation measured by blood tests
  • Reported new hip pain
  • Absence of swelling in the small joints of the hands and feet, and absence of positive blood tests for rheumatoid arthritis

The new classification criteria may also help to evaluate existing treatments for polymyalgia rheumatica.  Everyone with polymyalgia rheumatica is typically tested for temporal arteritis. This, too, would start with the exam and listening to the patient’s symptoms. If temporal arteritis is suspected, but less convincing features are present, a temporal artery biopsy may confirm the diagnosis. The biopsy is taken from a part of the artery located in the hairline, in front of the ear. In most cases the biopsy is helpful, but in some individuals it may be negative or normal, even though the person does have temporal arteritis.
