Acute Respiratory Distress Syndrome (ARDS) Overview
Acute respiratory distress syndrome (ARDS) is a serious life-threatening lung injury where fluid fills up the tiny, elastic air sacs (alveoli) in the lungs and this fluid leaks into the lungs. Too much fluid in the lungs can lower the amount of oxygen or increase the amount of carbon dioxide in the bloodstream. This deprives your organs of the oxygen they need to function. Breathing becomes difficult and oxygen cannot get into the body. ARDS can prevent the organs from getting the oxygen they need to function, and it can eventually cause organ failure. ARDS most commonly affects hospitalized patients who are very sick. It can also be caused by serious trauma. Symptoms usually occur within a day or two of the original illness or trauma, and they may include extreme shortness of breath and gasping for air.
ARDS typically occurs in patients who are already critically ill or who have significant injuries. Severe shortness of breath is the main symptom of ARDS, which usually develops within a few hours to a few days after the precipitating injury or infection. Many of the patients who develop ARDS don’t survive, the risk of death increases with age and severity of the illness. Other patients who do survive ARDS, some recover completely while others experience lasting damage to their lungs.
Signs and symptoms of Acute Respiratory Distress Syndrome (ARDS)
Difficulty breathing is usually the first symptoms of ARDS. Other signs and symptoms of ARDS may vary depending on the underlying cause and severity, as well as the presence of underlying heart or lung disease. ARDS may take several days to develop, or it can rapidly get worse.
Following are some of the signs and symptoms of acute respiratory distress syndrome:
- Severe shortness of breath
- Labored and unusually rapid breathing
- Low blood pressure
- Confusion and extreme tiredness
- Fast breathing, or taking lots of rapid, shallow breaths
- Fast heart rate
- Confusion
- Cough that produces phlegm
- Blue fingernails or blue tone to the skin or lips
- Fatigue
- Fever
- Crackling sound in the lungs
- Chest pain, especially when trying to breathe deeply
- Low blood pressure
Causes of Acute Respiratory Distress Syndrome (ARDS)
Damage to the lung’s air sacs called alveoli causes ARDS. Fluid from tiny blood vessels leaks through the damaged walls of the air sacs and collects, limiting the lungs’ normal exchange of oxygen and carbon dioxide. The damage also causes inflammation that leads to the breakdown of surfactant a liquid that helps keep the air sacs (alveoli) open. The air sacs may become damaged as a result of an illness, such as a lung infection, or breathing in smoke. Other illnesses or injuries may trigger inflammation that damages the air sacs.
Following are some of the underlying causes of acute respiratory distress syndrome (ARDS).
- Sepsis: The most common cause of ARDS is sepsis, a serious and widespread infection of the bloodstream.
- Inhalation of harmful substances: Breathing high concentrations of smoke or chemical fumes can result in ARDS, as can inhaling (aspirating) vomit or near-drowning episodes.
- Severe pneumonia: Severe cases of pneumonia usually affect all five lobes of the lungs.
- Head, chest or other major injury: Accidents, such as falls or car crashes, can directly damage the lungs or the portion of the brain that controls breathing.
- Coronavirus disease 2019 (COVID-19): Patients who have severe COVID-19 may develop ARDS.
- Others: Pancreatitis (inflammation of the pancreas), massive blood transfusions and burns.
